การประยุกต์ใช้กระบวนการสุนทรียสนทนาในการจัดการเรียนการสอนคุณค่าการเป็นพยาบาล : The Application of Dialogue in the Nursing Value Training Program
บทคัดย่อ
วัตถุประสงค์ : เพื่อพัฒนาการจัดการเรียนการสอนคุณค่าการเป็นพยาบาลโดยประยุกต์ใช้กระบวนการสุนทรียสนทนา และศึกษาการเปลี่ยนแปลงคุณค่าการเป็นพยาบาลของนักศึกษาพยาบาลที่ผ่านกิจกรรมการเรียนรู้
สถานที่ศึกษา : โรงพยาบาลสวรรค์ประชารักษ์ และวิทยาลัยพยาบาลบรมราชชนนีสวรรค์ประชารักษ์นครสวรรค์
รูปแบบการวิจัย : วิจัยคุณภาพแบบวิจัยเชิงปฏิบัติการ
กลุ่มตัวอย่าง : นักศึกษาพยาบาลชั้นปีที่ 3 รุ่นที่ 41 วิทยาลัยพยาบาลบรมราชชนนีสวรรค์ประชารักษ์นครสวรรค์ จำนวน 31 คน ที่ขึ้นฝึกปฏิบัติงานหอผู้ป่วยหนักอายุรกรรม ระหว่างวันที่ 1 ถึง 30 ตุลาคม 2555
วิธีการศึกษา : จัดกิจกรรมด้วยชุดการเรียนรู้ที่ประยุกต์ใช้กระบวนการสุนทรียสนทนาเป็นฐาน 10 ครั้งระหว่างวันที่ 1 ตุลาคม 2555 ถึงวันที่ 28 กุมภาพันธ์ 2556 เครื่องมือในการเก็บข้อมูลได้แก่บันทึกการเรียนรู้หลังทำกิจกรรม แบบบันทึกการสังเกต การบันทึกเสียง และแบบตรวจสอบรายการการเปลี่ยนแปลงพฤติกรรมที่เกิดขึ้นจริงของนักศึกษาพยาบาลตามแนวคิดกระบวนการปฏิบัติการด้วยความรักเพื่อการดูแลของวัตสัน สรุปเป็น 4 ประเด็นหลัก การเก็บข้อมูลประกอบด้วย 3 ส่วนโดยใช้กระบวนการตรวจสอบแบบสามเส้าด้านข้อมูล เปรียบเทียบมุมมองผู้เกี่ยวข้องได้แก่ ด้านผู้วิจัย ด้านนักศึกษาพยาบาล และอาจารย์พยาบาลที่เข้าร่วมสังเกตการณ์อย่างมีส่วนร่วม การวิเคราะห์ข้อมูลเชิงปริมาณนำเสนอเป็นค่าความถี่และร้อยละ และข้อมูลเชิงคุณภาพใช้การวิเคราะห์เนื้อหา
ผลการศึกษา : ข้อค้นพบจากการวิจัยพบว่า กระบวนการสุนทรียสนทนาที่ประยุกต์ขึ้นสามารถสร้างการเรียนรู้ภายในตัวนักศึกษาทำให้เกิดความไว้วางใจกัน มีการรับฟังอย่างลึกซึ้งไม่ด่วนตัดสิน เปิดใจในการแลกเปลี่ยนเรียนรู้ได้เห็นเรื่องราวของกันทั้งสุข ทุกข์ และแรงบันดาลใจ เกิดความคิดและความรู้สึกใหม่ๆขึ้นมาจนค่อยๆเปลี่ยนไปสู่คุณค่าที่มีต่อตนเองและผู้อื่น นักศึกษาเปรียบกระบวนการสุนทรียสนทนาเหมือน“ปั๊มน้ำมัน”ทุกคนเป็นรถที่มาเติมเชื้อเพลิงเป็นกำลังใจ ได้มาทบทวนตัวเอง ได้พูดความรู้สึกจริงๆด้วยบรรยากาศที่ผ่อนคลาย ได้ใคร่ครวญตัวเองเห็นความคิดฝันและมองถึงอนาคตของตนเองและเป็นกำลังใจให้กัน มุมมองอาจารย์พยาบาลเห็นว่ากระบวนการสุนทรียสนทนาเป็นเครื่องมือสำหรับอาจารย์ได้เรียนรู้ไปด้วยกันกับนักศึกษา ได้รู้จักนักศึกษาชัดขึ้น เข้าใจลูกศิษย์โดยไม่ตัดสินเพียงแค่เกรด เห็นคุณค่าความเป็นมนุษย์ในเด็กทุกคน เห็นลักษณะของการทำดีจากข้างในที่ปีติสุขและไปเกื้อกูลให้ผู้อื่นมีความสุข การประเมินตนเองของนักศึกษาพยาบาลจากแบบตรวจสอบรายการพบว่ามีการเปลี่ยนแปลงสู่คุณค่าการเป็นพยาบาลใน4 ประเด็นหลักทุกคน ได้แก่ประเด็นที่ 1 ไวต่อการรับรู้ความรู้สึกของตนเองและผู้อื่น พัฒนาในการรู้จักตนเองและยอมรับตนเองตามที่เป็นจริง ประเด็นที่ 2 ตระหนักรู้มีสติในการทำงานและการดำเนินชีวิตมากขึ้น ประเด็นที่ 3 สร้างสัมพันธภาพ การช่วยเหลือได้อย่างไว้วางใจ และประเด็นที่ 4 การปฏิบัติด้วยความรักความเมตตาต่อตนเองและผู้อื่น
วิจารณ์และสรุป : การจัดชุดการเรียนรู้ที่ประยุกต์ใช้กระบวนการสุนทรียสนทนาเป็นฐานทั้ง 10 ชุด เป็นเครื่องมือสำหรับอาจารย์ได้เรียนรู้ไปด้วยกันกับนักศึกษาพยาบาล กระบวนการสุนทรียสนทนาช่วยกล่อมเกลาให้นักศึกษาเข้าสู่การดูแลเอื้ออาทรซึ่งเป็นคุณค่าของการพยาบาล

Abstract
Objective : To develop teaching method of nursing value by using dialogue and to study changes in nursing value in nursing student.
Setting : Sawanpracharak Hospital, Boromarajonani College of Nursing Sawanpracharak Nakhonsawan.
Design : Qualitative Research, Action Research.
Subject : 31 3rd year nursing students at Boromarajonani College of Nursing, Sawanpracharak Nakhonsawan who were trained in Medicine ICU. ward during October, 2012.
Method : The students attended of 10 sessions of applied process of dialogue during 1st October 2012 to 28th February 2013. The instruments used for evaluation included student after activity journal report of observation of activity and self assessed record of a check list outlining the changes in behavior. It based on Watson criteria. The data were collected from the researcher, the nursing students, and participating nursing teachers then, the information gathered were checked and point of views were compared sections by checking three pillars of information and comparing the points of view of those involved including: For quantitative and qualitative analysis percentage and analysis of content were used
Result : The application of dialogue in nursing value training program could build the inner learning process of nursing students through trust, deep listening, open their heart to share, and welcome stories of happiness, sufferance, and inspiration. The experience of new ideas and feeling will slowly transform the students. The nursing students compare dialogue process as “a fuel pump”. Everyone was like a car which came to fuel up for spirit, they were able to review themselves and speak their true feeling in a relaxing environment. They could contemplate upon themselves and see their own dreams, and aim, and encourage each other. As for the teachers the dialogue process was the tool to enable teachers to learn with the students, to know and understand the students more clearly and not to judge the students only on their grades. They could see the humanity value in every student and see the good deed coming from inner joyful spirit. From self assessment the students reported; 1) self awareness and sensitivity to other acceptance of one’s own actualization. 2) Awareness, and more conscious of work and life. 3) Developing and sustaining a better, helping-trusting authentic caring relationship. 4) Practice with loving kindness for oneself and others.
Conclusion : The application of 10 basis of dialogue was the tool to enable teachers to learn along with students. The dialogue process could build the inner learning process of nursing students, and resulting in a change in the value of nursing.
