วัตถุประสงค์ : เพื่อวิเคราะห์ปัญหา กำหนดแนวทาง และสร้างความร่วมมือของภาคีเครือข่ายในการพัฒนาการจัดการโรคหลอดเลือดสมอง
สถานที่ศึกษา : ตำบลวังยาง อำเภอคลองขลุง จังหวัดกำแพงเพชร
รูปแบบการวิจัย : การวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม
กลุ่มตัวอย่าง : ทีมแกนนำระบบสุขภาพอำเภอคลองขลุงคณะกรรมการพัฒนาคุณภาพชีวิตระดับอำเภอคลองขลุงและภาคีเครือข่ายอาสาสมัครสาธารณสุขและประชาชนตำบลวังยางอำเภอคลองขลุง
วิธีการศึกษา : ขั้นศึกษาบริบท ทำการศึกษาข้อมูลทุติยภูมิเกี่ยวกับสถานการณ์ปัญหาและบทบาทของผู้เกี่ยวข้องในการป้องกันและแก้ไขปัญหาโรคหลอดเลือดสมอง ขั้นวิเคราะห์ ทำการวิเคราะห์ปัญหาทำความเข้าใจถึงสาเหตุที่แท้จริงว่าการมีส่วนร่วมของแต่ละภาคส่วนมีปัญหาหรืออุปสรรคใด เพื่อนำไปกำหนดแผนยุทธศาสตร์แบบมีส่วนร่วมในการป้องกันโรคหลอดเลือดสมองโดยมีการนำเสนอผ่านมติคณะกรรมการพัฒนาคุณภาพชีวิตระดับอำเภอและขั้นการนำแผนงานไปดำเนินการในพื้นที่ตำบลวังยางอำเภอคลองขลุงโดยมุ่งเน้นการสร้างกระบวนการมีส่วนร่วมและเฝ้าติดตามผลของการมีส่วนร่วม
ผลการศึกษา : ขั้นศึกษาบริบทพบว่าผู้ป่วยโรคหลอดเลือดสมองแบบเฉียบพลันยังไม่สามารถเข้าถึงระบบบริการฉุกเฉินได้อย่างมีประสิทธิภาพ ระบบการส่งต่อฉุกเฉินไม่ครอบคลุมในระดับอำเภอและประชากรกลุ่มเสี่ยงที่ป่วยด้วยโรคความดันโลหิตสูงและเบาหวานสูงขึ้นอย่างต่อเนื่อง ขั้นวิเคราะห์พบว่าส่วนใหญ่เคยรับรู้เกี่ยวกับโรคหลอดเลือดสมอง ซึ่งแสดงถึงหลอดเลือดสมอง แตก ตีบ ตันหรือโรคอัมพฤกษ์/อัมพาตโดยมีอาการแขนขาอ่อนแรงแต่สามารถรักษาให้หายได้และขั้นการนำแผนงานไปดำเนินการพบว่ามีการพัฒนาระบบการส่งต่อผู้ป่วยฉุกเฉินโรคหลอดเลือดสมองการพัฒนาศักยภาพภาคีเครือข่ายและชุมชนเพื่อสร้างการตระหนักรู้และการมีส่วนร่วมของทุกภาคส่วนสู่การปฏิบัติและการพัฒนาระบบบริหารและระบบสนับสนุนการทำงาน
วิจารณ์และสรุป : การศึกษานี้แสดงให้เห็นถึงผลลัพธ์ที่เกิดจากกระบวนการดำเนินงานที่เกิดจากการมีส่วนร่วมของทุกภาคส่วนในพื้นที่ตำบลวังยางในการแก้ไขและป้องกันปัญหาโรคหลอดเลือดสมองจนเกิดแนวทางปฏิบัติและมีการขยายผลไปยังพื้นที่ตำบลข้างเคียง

Abstract

Objective : In order to analyze the problem, set guidelines and create cooperation among network partners in the development of stroke management.
Setting : Wang Yang sub-district, KhlongKlung district, Khampangphet province
Design : Participatory action research design
Subjects : District health management team, KhlongKhlung district health board, other key stakeholders such as leaders of local governments, private sectors, residents, and village health volunteers
Methods : First phase, data on situations, problem, and role of persons ours involved in prevention and resolution of stroke problem are reviewed. Second phase, factors associate with participation of each sector in the prevention of stroke are analyzed. Third phase, strategic plan is formulate base on factors devived from 2nd and plase and implemented at Wang yang sub-district, KhlongKlung district, by focusing on creating participation process and monitoring the result of participation.
Results : The patients with acute stroke attack could not get an effective access to emergency services, referral system for emergency coverage at the sub-district level and the number of population at risk of hypertension and diabetes mellitus was rising steadily. Most of them know that stroke caused atresia/paralysis that can be cured. An emergency stroke system by associating network, capacity building for community partnership networks and people to build awareness and participation of all sectors into practice, and development, management and operation support systems were developed.
Conclusion : This study shows the preliminary results from the implementation of the participation of all sectors in the Wang yang sub-districts to solve and prevent stroke, as a result practice guideline was established and the practice were extended to neighboring sub-districts areas.
