Abstract

This study aims to develop home health care system for patient with continuing care needs using community participation approaches. Between 2013 and 2016, using the concept of palliative care patients, the WHO 6 concept is oriented content. The action research and participatory framework is to drive the development process. Screw the development process every year, until the system and the results are satisfactory. A total of 290 patients were divided into three target groups (dependent elders, disabled people and palliative care patients). Home health care services were conducted by public health team and continuing care volunteer (n=96). The quantitative data was analyzed using percentage and the qualitative data was analyzed using content analysis. The findings show that the home health care system for patient with continuing care needs using community participation approaches consist of (1) building humanized health care network (public health team and continuing care volunteer), (2) integrated health care services and teams, (3) setting-up the care coordination center in sub district level, (4) developing district guidelines forcare coordination center, (5) supporting the empowerment of health care network to cultivate the seeds of goodness, (6) setting-up the COC district center for complex continuing care patients, (7) continuously monitoring and evaluation by using knowledge sharing forum, and (8) supporting COC systems such as equipment, funding and data base system of continuing care patient. This approach could improve patient access to continuing care services and increase their quality of life, satisfaction and good death.
