บทคัดย่อ

               The objective of this study was to analyze prognostic factors associated diabetes mortality and the duration of hospital stay for the major caused of diabetic admissions among patients admitted to medical wards of Ban-Mi hospital, Lopburi province, Thailand. It was conducted as a retrospective descriptive study during January-December 2015. There were altogether 3,352 patients registered at Medical ward of the hospital, and 485 of them were diagnosed as diabetes, of whom 19 cases refused treatment or were referred, resulting in 466 diabetic patients with completed treatment and were included in this study. Relevant data relating to the hospital stay, underlying conditions, and treatment outcomes were collected and analyzed. It was found that diabetes related admission were 14.5% and the fatality rates 16.7% (78 patients). Diabetic admissions lasted longer than the average hospital admissions (7.00±10.62 vs 5.33±6.76 days) p<0.001 whether due to control of diabetes or manage of complications. Diabetic foot ulcer had the most prolong duration of admission 27.20±34.86 days, ranging from 4-130 days. Reasons for hospital-ization were acute metabolic complication 33.4% (162 patients), and infection 30.7% (149 patients). The most common associated condition was hypertension which was presented in 37.9% of the cases (184 patients). The most common causes of deaths were septicemia, 43.6% (34 patients); cardiovascular disease, 14.1% (11 patients); pneumonia, 12.8% (10 patients); and cerebrovascular disease, 9.0% (7 patients). Cirrhosis and septicemia had the highest case fatality rate (56.0% and 48.0%, respectively). However, after multivariate analysis we found that septicemia, cardiovascular disease, pneumonia and chronic kidney disease were prognostic factors significantly associated with the diabetes mortality.
