THE DEVELOPMENT OF PHARMACEUTICAL CARE FOR CHRONIC KIDNEY DISEASE PATIENTS IN THE SLOW PROGRESSION CHRONIC KIDNEY DISEASE CLINIC, PHATTHALUNG HOSPITAL

This research aimed to study medication use related problems, evaluate, and develop a pharmaceutical care model for chronic kidney disease patients in the slow progression chronic kidney Disease Clinic, Phatthalung Hospital. Method: This study using a three - phase research and development process: (1) Studied the incidence of medication use related problems and the current pharmaceutical care model in 90 CKD participants. (2) Development of a new pharmaceutical care model for CKD patients. (3) Evaluation of the developed pharmaceutical care model in 60 participants. Data were collected using questionnaires. General data were analyzed using Percentage, Mean, and Standard Deviation. Clinical Outcomes and Patient Outcomes before and after the intervention was compared using the paired t-test. Results: In Phase 1, a total of 69 medication use related problems were identified (76.67%). The most common issue was medication non-compliance 35 problems (38.89%). The second most common issue was improper medication use selection 18 problems (20.00%). Phase 2 resulted in the development of a full scale pharmaceutical care model for CKD patients (CKD-PTL MODEL) with a outpatient pharmaceutical care recording program and a pharmaceutical care referral system. In Phase 3, after implementing the CKD-PTL model found that the sample group had a statistically significant increase in mean glomerular filtration rate (eGFR) (p=0.007). Mean of medication adherence, knowledge about medications, and understanding of chronic kidney disease significantly improved (p<0.01, p<0.01, p< 0.01). In addition, the number of problems involving irrational medication use decreased from 50 problems (83.33%) to 19 problems (31.67%). Conclusion: The development of the pharmaceutical care model for chronic kidney disease patients enhanced medication knowledge and adherence to correct medication use, and reduced problems related to irrational medication use. This can help slow progression in CKD disease patients.
