Maternal and Neonatal Outcomes Among Pregnant Women with Gestational Diabetes Mellitus and Hypertensive Disorders of Pregnancy at Warinchamrab Hospital, Ubon Ratchathani Province

Abstract

Objective: To compare maternal and neonatal outcomes between pregnant women with Gestational Diabetes Mellitus (GDM) combined with Hypertensive Disorders of Pregnancy (HDP) and women with normal pregnancies.

Methods: This retrospective cohort study included pregnant women who delivered at Warinchamrab Hospital, Ubon Ratchathani Province, between January 2019 and December 2024. A total of 274 women were enrolled, comprising 137 women with GDM+HDP and 137 women with normal pregnancies. Maternal outcomes and Neonatal outcomes included hypoglycemia, hyperbilirubinemia, respiratory distress, and other complications. Descriptive statistics, Risk Ratio (RR), and Adjusted Risk Ratio (aRR) were calculated to compare outcomes between groups.

Results: Maternal outcomes that differed significantly between the GDM+HDP and normal pregnancy groups included cesarean delivery (46.0% vs. 16.1%; p-value<0.01), postpartum hemorrhage (33.6% vs. 15.3%; p-value<0.01), and preterm birth (18.2% vs. 7.3%; p-value<0.01). Regarding neonatal outcomes, the GDM+HDP group had higher incidences of neonatal hypoglycemia (37.2% vs. 17.5%; p-value<0.01), hyperbilirubinemia (39.4% vs. 24.1%; p-value<0.01), and respiratory distress (51.8% vs. 23.4%; p-value<0.01). After adjustment for confounders, GDM combined with HDP was significantly associated with increased risks of cesarean delivery (aRR 2.19; 95% CI 1.34–3.56), postpartum hemorrhage (aRR 2.10; 95% CI 1.23–3.56), preterm birth (aRR 2.59; 95% CI 1.17–5.74), and neonatal respiratory distress (aRR 2.11; 95% CI 1.43–3.12).

Conclusion: Pregnant women with GDM combined with HDP have significantly higher risks of adverse maternal and neonatal outcomes compared with women with normal pregnancies. Close monitoring and comprehensive perinatal care are essential for this high-risk population.
