การศึกษาการมีส่วนร่วมของภาครัฐ ท้องถิ่น และชุมชน ในการดูแลสุขภาพผู้สูงอายุ ที่อาศัยลำพังในเขตเทศบาลนคร นครสวรรค์ : Education the participation in local government and local community to supervise unaided elder’s health care in Nakhonsawan municipal area

Abstract

วัตถุประสงค์ :เพื่อศึกษาสถานการณ์การดูแลสุขภาพผู้สูงอายุที่อาศัยลำพังและเพื่อศึกษาการสนับสนุนการดูแลสุขภาพผู้สูงอายุที่อาศัยลำพังของภาครัฐ ท้องถิ่น และชุมชน

สถานที่ศึกษา : ชุมชนในเขตเทศบาลนครนครสวรรค์ ที่สมัครใจเข้าร่วมโครงการ จำนวน 6 ชุมชน

รูปแบบการวิจัย : การวิจัยเชิงพรรณนา (descriptive research)

กลุ่มตัวอย่าง : ผู้สูงอายุที่อาศัยลำพังใน 6 ชุมชน โดยคัดเลือกตามประเภทการอยู่อาศัย จำนวน 388 คนและผู้บริหารหรือผู้รับผิดชอบงานผู้สูงอายุจากโรงพยาบาลสวรรค์ประชารักษ์ เทศบาลนครนครสวรรค์ ศูนย์สุขภาพชุมชนเมือง ศูนย์บริการสาธารณสุขเทศบาลนครนครสวรรค์ประธานชมรมผู้สูงอายุ ตัวแทนอาสาสมัครประจำหมู่บ้าน ผู้สูงอายุที่มีบทบาทในชุมชน และผู้สูงอายุที่มีภาวะพึ่งพิง รวมจำนวน 20 คน

วิธีการศึกษา : เก็บข้อมูลโดยใช้ 1) แบบสำรวจผู้สูงอายุที่อาศัยลำพัง ประกอบด้วยข้อมูลทั่วไป สภาวะแวดล้อมการรับรู้และการเข้าถึงสิทธิตามมาตรฐาน ข้อมูลด้านสุขภาพ และข้อมูลความต้องการการช่วยเหลือใน 7 ด้าน 2) แบบประเมินสุขภาพจิต 3) แบบประเมิน ADL และ 4) แบบสำรวจการสนับสนุนการดูแลผู้สูงอายุที่อาศัยลำพัง วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา เป็นค่าความถี่ ร้อยละ และการวิเคราะห์เนื้อหา

ผลการศึกษา : ผู้สูงอายุที่อาศัยลำพัง เป็นเพศชาย ร้อยละ 67.3 เป็นผู้สูงอายุวัยต้น อายุ 60 - 69 ปีร้อยละ 55.9 อยู่ลำพังเฉพาะตอนกลางวันมากที่สุดร้อยละ 41.2 มีรายได้น้อยกว่า 1,500 บาท ต่อเดือนมากที่สุดร้อยละ 20.6 ด้านสิ่งแวดล้อม พบว่าไม่มีราวจับในส้วมร้อยละ 77.8 การรับรู้และการเข้าถึงสิทธิตามมาตรฐานการส่งเสริมสวัสดิภาพ พบว่าการเป็นสมาชิกชมรมผู้สูงอายุมีน้อยที่สุดเพียงร้อยละ 11.1 ด้านสุขภาพพบว่ามีโรคประจำตัวร้อยละ 75.0 โดยโรคที่พบมากที่สุด คือความดันโลหิตสูงร้อยละ 71.8 ด้านความต้องการให้ช่วยเหลือ พบว่าต้องการให้ครอบครัวช่วยเหลือมากที่สุดทั้ง 7 ประเด็นหลัก สำหรับกิจกรรมที่มีการสนับสนุนจากภาครัฐ ท้องถิ่น และชุมชน สามารถตอบสนองความต้องการได้ทุกประเด็น

สรุป : พบว่าผู้สูงอายุอยู่ในสังคมเขตเมือง แต่ยังมีบ้านญาติ หรือเพื่อนบ้านที่อยู่ใกล้เคียงให้การช่วยเหลือกันได้เป็นจำนวนมาก ซึ่งแสดงให้เห็นว่าภายในชุมชนยังมีความผูกพัน และความเอื้อเฟื้อเผื่อแผ่ที่จะช่วยเหลือกัน นอกจากนั้นส่วนใหญ่ยังมีครอบครัวในการสนับสนุนช่วยเหลือและคอยดูแล สำหรับภาครัฐ ท้องถิ่นและชุมชนมีการจัดกิจกรรมที่ตอบสนองกับความต้องการของผู้สูงอายุ แต่ยังไม่เพียงพอเพราะจากข้อมูลยังมีปัญหาเรื่องรายได้สิ่งแวดล้อมในบ้าน การจัดตั้งชมรมผู้สูงอายุ และภาวะเจ็บป่วยด้วยโรคเรื้อรัง ในอนาคตจึงต้องมีการดูแล และสนับสนุนจากภาครัฐ ท้องถิ่น ชุมชน และภาคส่วนต่างๆ ที่เกี่ยวข้องอย่างบูรณาการร่วมกัน

Abstract

Objectives : To study the situation for taking care of unaided elders health and study the health service supporting system provided by the government and local community.

Setting : 6 communities in Nakhonsawan Municipality interested in joining the project.

Design : Descriptive Research

Samples : 388 unaided elders from 6 communities were selected by residential category. Besides, the director or personnel in change of elders from Sawanpracharak Hospital, Nakhonsawan Government, Primary Care Unit, Nakhonsawan Municipality Public Health Center, a leader in elders club, village volunteer representative, elders in the community and 20 dependency elders were included.   

Methods : Data collected by 1) questionnaire 2) The mental health assessment 3) ADL assessment 4) The survey of supporting health service for unaided elders. Qualitative method was used to analize into frequency, percentage and content analysis.

Results : 67.3% of the elders was male and 55.9% were in their sixties. 41.2% was alone in day time. 20.6% earning less than 1500 bath a month. Environmental aspect, 77.8% of them had no handrails in restroom. Recognition and accession to the basic rights for welfare promotion found only 11.10% belonged to the elder club’s member. 78.8% of them was found having congenital disease: hypertension 71.8%. The family members were needed to assist them in 7 aspects. The activities provided by the government, local community can respond to every demand

Conclusion : The situation of unaided elders caring in the city showed the bonding and generosity among the elders in communities by the assistance of both their relatives and neighbours. Thus, The plan to support them would be better in near future, then the situation will cause the caregiver to pay more attention to them. For the government, local and community, the activities provided were not enough to respond their demands, However, The were some problems such as outcome, housing environment, elder’s club establishment and sickness from chronic diseases, hence, the assistance from the government and local, communities were also needed.
