การบำบัดทางทันตกรรมจัดฟันก่อนการศัลยกรรมสำหรับทารกปากแหว่งเพดานโหว่ ในโรงพยาบาลสวรรค์ประชารักษ์:Presurgical Orthopedic Therapy of Cleft Lip and Palate Infants in Sawanpracharak Hospital

Abstract

     เพดานเทียมเป็นเครื่องมือที่ใช้ช่วยแก้ปัญหาการดูดนม และใช้ในการจัดเรียงสันกระดูกขากรรไกรบนให้อยู่ในตำแหน่งที่ใกล้เคียงกับตำแหน่งที่ปกติก่อนการผ่าตัด ในทารกที่มีภาวะปากแหว่งเพดานโหว่ ผลดีของการรักษาด้วยเพดานเทียมยังช่วยให้การทำศัลยกรรมตกแต่งบริเวณริมฝีปากดีและง่ายขึ้น อีกทั้งยังลดความรุนแรงของการสบฟันที่ผิดปกติด้วย

     บทความนี้ได้นำเสนอรายงานการรักษาผู้ป่วยปากแหว่งเพดานโหว่ 1 ราย เป็นทารกเพศชาย อายุ 3 วัน มีภาวะปากแหว่งเดพานโหว่ด้านซ้ายแบบสมบูรณ์ ได้รับการรักษาด้วยเพดานเทียมชนิดที่ไม่มีแรงกระทำและชนิดที่มีแรงกระทำก่อนทำการผ่าตัดริมฝีปาก 65 วัน ผลการรักษาเป็นที่พอใจ ช่องโหว่ของเพดานมีขนาดลดลง ผู้ปกครองพอใจและมีสุขภาพจิตที่ดีขึ้น ผู้ป่วยดูดนมได้ดี

Abstract

     Obtulator is an appliance of choice for solving feeding problem and aligning upper alveolar ridges prior to surgical correction in cleft lip and palate baby. Obtulator also has an added advantages of improving surgical out come and simplifying lip and decreasing severity of malocclusion

     A case report of a 3-day-old baby boy with unilateral left complete cleft lip and palate being treated with passive obtulator and active obtulators for 2 month prior to lip repair is presented. The treatment outcome is entirely satisfactory. Patient has good feeding closure of cleft palate also brings parents great satisfaction and peace of mind.
