การจัดการฟันตัดหน้าบนแท้ที่ถูกดันเข้าเบ้าฟันหลังได้รับอุบัติเหตุเป็นเวลานานร่วมกับมีภาวะแทรกซ้อนของรากฟันละลาย : Management of delayed repositioning of intruded permanent maxillary incisors with root resorption as complication

Abstract

บทคัดย่อ : อินทรูชันหรือฟันถูกดันเข้าเบ้าฟันเป็นการบาดเจ็บที่เกิดขึ้นกับฟันที่มีความรุนแรงที่สุด เนื่องจากทำอันตรายต่อเนื้อเยื่อใน เอ็นยึดปริทันต์และกระดูกเบ้าฟัน ทำให้เกิดการตายของเนื้อเยื่อใน รากฟันละลาย รวมทั้งเกิดการละลายของกระดูกเบ้าฟันส่วนขอบตามมา การรักษาจึงมีความยุ่งยากซับซ้อน การประเมินความสำเร็จของการรักษาจึงจำเป็นต้องได้รับการติดตามผลอย่างต่อเนื่อง
รายงานผู้ป่วยที่ฟันได้รับอุบัติเหตุมานาน 3 เดือน ฟันตัดบนซ้ายถูกดันเข้าไปในเบ้าฟันร่วมกับฟันแตก ฟันตัดด้านขวาเคลื่อนขยับ ลักษณะทางคลินิกและภาพถ่ายรังสีพบการละลายของรากฟัน ผู้ป่วยได้รับการรักษาโดยดึงฟันกลับมาตำแหน่งเดิมด้วยวิธีทางศัลยกรรม และรักษาคลองรากฟัน โดยใส่แคลเซียมไฮดรอกไซด์เป็นเวลานาน และบูรณะฟันที่หักด้วยวัสดุสีเหมือนฟัน จากการติดตามผลการรักษาเป็นเวลา 3 ปี ผู้ป่วยไม่มีอาการปวด ฟันสามารถอยู่ในช่องปากใช้งานได้ และผู้ป่วยยอมรับได้

Intrusive luxation is the most severe type of traumatic dental injury because it significantly damages the pulp, periodontal ligament and alveolar bone. The evaluation of treatment success needs long follow-up period.
This paper reports a case of delayed repositioning of severely intruded permanent left maxillary incisors accompanied by uncomplicated crown fracture and right maxillary luxation after 3 months. Clinical and radiographic examination showed root resorption. The teeth were surgically reposition and endodontic treatment with long term calcium hydroxide medicament. The teeth were restored with resin composite. During 3 year follow-up patient did not complain have pain. The teeth had remained functional and esthetically acceptable.
