การประเมินประสิทธิภาพชุดตรวจเอชไอวี รุ่นที่ 4 ชนิดได้ผลเร็ว เปรียบเทียบกับเครื่องตรวจวิเคราะห์อัตโนมัติ Evaluate the Effective of HIV Fourth Generation Rapid Test Compare with Automation

Abstract

วัตถุประสงค์          : เพื่อศึกษาประสิทธิภาพชุดตรวจเอชไอวี รุ่นที่ 4 ชนิดได้ผลเร็วเปรียบเทียบกับเครื่องตรวจวิเคราะห์อัตโนมัติ

สถานที่ศึกษา          : ห้องปฏิบัติการทดสอบพิเศษ กลุ่มงานเทคนิคการแพทย์ โรงพยาบาลสวรรค์ประชารักษ์ 

รูปแบบงานวิจัย     : การศึกษาวิจัยเชิงพรรณนาแบบไปข้างหน้า

กลุ่มตัวอย่าง           : ผู้ป่วยนอกที่มารับบริการตรวจเอชไอวี ทางห้องปฏิบัติการ ที่มีใบสั่งตรวจจากแพทย์ไม่จำกัดอายุ เพศ และอาการ ตั้งแต่ 13 ธันวาคม 2562 ถึง 16 เมษายน 2563 จำนวน 420 ราย

วิธีการศึกษา            : เก็บข้อมูลพร้อมบันทึกผลการตรวจวิเคราะห์เอชไอวี จากเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4และชุดตรวจเอชไอวีรุ่นที่4ชนิดได้ผลเร็วด้วยตัวอย่างเดียวกันตามแนวทางคู่มือการตรวจหาการติดเชื้อเอชไอวีสำหรับเทคนิคการแพทย์วิเคราะห์ร้อยละผลบวกร้อยละผลลบ และผลต่างของร้อยละผลบวกและผลลบของทั้งสองวิธี พร้อมนำตัวอย่างที่ให้ผลต่างกันของทั้งสองวิธีไปตรวจวิเคราะห์สารพันธุกรรมด้วยวิธีพีซีอาร์ และทดสอบทางสถิติหาความสอดคล้องหรือแตกต่างของทั้งสองวิธีด้วย Pearson Chi-Square

ผลการศึกษา           : ร้อยละของผลบวกจากเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4กับชุดตรวจเอชไอวี รุ่นที่ 4ชนิดได้ผลเร็วคิดเป็นร้อยละ 25.7 และร้อยละ 22.6 และร้อยละผลลบจากเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4กับชุดตรวจเอชไอวี รุ่นที่ 4ชนิดได้ผลเร็วคิดเป็นร้อยละ 74.3 และร้อยละ 77.4 ผลต่างร้อยละของผลบวกและผลลบที่ต่างกัน ของทั้งสองวิธีเท่ากับ 3.1เมื่อนำตัวอย่างที่ให้ผลต่างกันของทั้งสองวิธีตรวจวิเคราะห์จากสารพันธุกรรมด้วยวิธีพีซีอาร์ พบผลบวกปลอมจากเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4ทั้งหมด

วิจารณ์และสรุป     : เครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4 อาจจะให้ผลบวกปลอมได้ในบางตัวอย่างที่มีการเตรียมสิ่งส่งตรวจไม่เหมาะสมชุดตรวจเอชไอวี รุ่นที่ 4ชนิดได้ผลเร็วสามารถใช้เสริมหรือทดแทนเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4 ได้แต่ชุดตรวจเอชไอวี รุ่นที่ 4ชนิดได้ผลเร็วมีราคาแพงกว่าเครื่องตรวจวิเคราะห์อัตโนมัติน้ำยาตรวจรุ่นที่ 4จึงอาจจะต้องพิจารณาในการเลือกใช้ต่อไป

Abstract

Objective                : To study the effectiveness of HIV 4^th generation rapid test compare with automatic analyzer

Setting                    : Special lab department of Sawanpracharak hospital

Design                     : Prospective descriptive research

Subject                    : Outpatient hospital Sawanpracharak doctor ordered HIV any gender, age, illness, during 13 December 2019 – 16 April 2020 of 420 samples.

Method                   : Collect data and record the results of HIV analysis from the 4th Generation Reagent automatic analyzer  and HIV 4th generation rapid test based on the same samples, according to the Guide to HIV Testing for Medical Technique.The results were statistically tested as percentage, sum, percentage, negative, difference of percentage, positive and negative of both methods.The samples were also used to analyze the genetic material by PCR Techniques and statistical tests, to find the consistency or difference of both methods using Pearson chi-square.

Result                      : Based on statistical test, percentage of positive result from 4^th generation reagent Automation machines and HIV 4^th generation rapid testpercentage 25.7 and 22.6 and the negative percentage from the from 4^th generation reagent automation machines and HIV 4^thgeneration rapid test percentage 74.3 and 77.4,difference of percentage, positive and negative of both methods as 3.09.When the samples differed from the two methods were used for the PCR Techniques the results were obtained as follows providing test results accordingly giving test results according to HIV 4^th generation rapid test for every sample.

Conclusion            : The 4^th generation reagent automationmachines to detect may produce a false positive result in some samples with improperly prepared specimens. HIV 4^th generation rapid test can be used as a supplement or substitute 4^thgeneration reagent automationmachines. But with the price of HIV 4th generation rapid test is relatively more expensive than 4^th generation reagent automation machines may need to be assembled in the selection.
