Giant cell rich osteosarcoma และความท้าทายในการวินิจฉัย ทางจุลพยาธิวิทยา : รายงานผู้ป่วย

บทคัดย่อ

Giant cell rich osteosarcoma เป็น Rare subtype ของ Conventional osteosarcoma พบได้ร้อยละ 1-3 มีลักษณะทางจุลพยาธิวิทยาคือ พบ Giant cell ประกอบด้วย Non-neoplastic osteoclast-like giant cell (No nuclear atypia) และ Pleomorphic malignant giant cell จำนวนมาก กระจายและแทรกตัวใน Mononuclear neoplastic cell ทีมีลักษณะ High pleomorphism ซึ่งสร้าง Osteoid หรือ Bone matrix ลักษณะทางจุลพยาธิวิทยาดังกล่าวคล้ายกับ Giant cell tumor of bone แตกต่างกันตรงที่ Giant cell tumor จะไม่พบ Osteoid หรือ Bone matrix ส่วนใน Malignant in giant cell tumor จะพบ Malignant component ที่เป็น Osteosarcoma และมีการสร้าง Osteoid หรือ Bone matrix ได้ มีลักษณะทางจุลพยาธิวิทยาเหมือนกับ Giant cell rich osteosarcoma ดังนั้นอาจเกิดความสับสนในการวินิจฉัยทางพยาธิวิทยา บทความได้นำเสนอตัวอย่างของ ผู้ป่วยที่มาด้วยก้อนที่หัวเข่าข้างขวา ผลตรวจทางรังสีวิทยาพบ Osteolytic lesion at epiphysis and metaphysis of distal femur วินิจฉัยทางพยาธิวิทยาจาก Biopsy specimen ว่า Giant cell tumor of bone และวินิจฉัยทางพยาธิวิทยาใน Wide resection specimen เป็น Malignant in giant cell tumor หลังจากทบทวนลักษณะทางพยาธิวิทยาได้เปลี่ยนการวินิจฉัยเป็น Giant cell rich osteosarcoma
