The Results of Care for Patients after Receiving Total Hysterectomy at Sichon Hospital

This study was a retrospective cohort study that aimed to study the results of care for patients after receiving total hysterectomy at Sichon Hospital. By studying the medical records of patients who had a total hysterectomy from the year 2021-2023, the sample consisted of 232 patients in the entire population (Census), divided into groups that received a total abdominal hysterectomy amount 206 pregnant women and 26 women who had a total vaginal hysterectomy. Using a data recording form developed based on literature review and validated through quality assessment of the tool with a CVI value of 0.93. The results of care for both groups of patients were analyzed using an independent t-test and Fisher's extract test. The results of the study found that the patients had an average age of 49.43 ± 10.02 years, an average body mass index of 26.46 ± 4.84 kg/m2, 61.2% had a history of abdominal or pelvic surgery, 75% had a history of childbirth. The most common diagnosis was Myoma Uteri at 44.8 %, followed by Adenomyosis and Prolapse Uteri at 26.7% and 11.2%, respectively. Most had experienced an operation time of less than two hours, and 76.7% had an amount of blood loss less than 500 cc. approximately 83.6%. Patients received blood during surgery 14.7% and had joint surgery 78.4%. Regarding the results of care, it was found that patients had an average length of hospital stay of 5.61 ± 3.67 days, and the type of discharge improved by 100%. Most patients did not develop any complications during and after surgery accounting for 92.2% and 94.8%, respectively. The study found that the length of hospital stays, and complications during and after surgery was no statistical difference between the two groups. When comparing the results of complications during and after surgery, it was found that there was a statistically significant difference between the two groups (P<0.05). In Patients who undergo abdominal hysterectomy will have blood loss and injury to nearby organs during surgery and after surgery there was postoperative bleeding and surgical wound infection. On the other hand, patients who had vaginal hysterectomies did not experience such complications. The results of this study demonstrate the results of patient care after receiving total abdominal hysterectomy and vaginal hysterectomy are not different. However, guidelines for caring for patients should be developed to be more efficient to reduce the length of hospital stay and reduce complications during and after surgery in total abdominal hysterectomy.
