The effect of promoting family support program through LINE application on physical activity adherence in persons with chronic obstructive pulmonary disease

Abstract

            Chronic obstructive pulmonary disease (COPD) limits physical activity and is linked to higher all-cause mortality, yet most patients remain physically inactive. This quasi-experimental study compared physical activity adherence between COPD patients receiving a family support promotion program via the LINE application and those receiving usual nursing care. Participants were 48 patients aged 40 years and older with GOLD stage 1-2 severity, purposively selected, matched by sex, age, and disease severity, and allocated into experimental and control groups (n=24 each). The experimental group received a 6-week program based on House's social support theory, covering emotional, informational, appraisal, and instrumental support through LINE. Physical activity cooperation was measured using the Global Physical Activity Questionnaire (GPAQ) and analyzed with descriptive statistics and independent t-tests.

            Baseline physical activity did not differ between groups (292.50 vs 306.67 METs-min/week, p=.315). After 6 weeks, the experimental group scored significantly higher than the control group (604.17 vs 348.33 METs-min/week; mean difference 255.84, 95% CI: 177.55, 334.13; t(46)=6.577, p<.001; Cohen's d=1.90), shifting from insufficient to sufficient activity level, while the control group remained insufficient.

            The family support program through LINE significantly improved physical activity adherence in COPD patients, both statistically and clinically. Nurses may integrate it into usual care, with early family preparation. Larger, multi-site studies with longer follow-up are recommended to confirm sustainability.
