การพัฒนาศักยภาพแกนนำครอบครัวในการเฝ้าระวังภาวะสมองเสื่อมของผู้สูงอายุร่วมบ้าน

ภาวะสมองเสื่อมเป็นปัญหาสาธารณสุขสำคัญที่ส่งผลกระทบรุนแรงต่อครอบครัวผู้สูงอายุ โดยเฉพาะในตำบลนาฝาย จังหวัดชัยภูมิ ที่แกนนำครอบครัวขาดศักยภาพในการเฝ้าระวังและดูแล การวิจัยนี้มีวัตถุประสงค์1) ศึกษาสถานการณ์และความต้องการของแกนนำครอบครัว 2) พัฒนารูปแบบการพัฒนาศักยภาพ และ 3) ทดลองใช้รูปแบบ กลุ่มตัวอย่างประกอบด้วยผู้สูงอายุ 306 คน แกนนำครอบครัว 35 คน และผู้เกี่ยวข้อง 43 คน รูปแบบพัฒนาผ่านกระบวนการทบทวนวรรณกรรม ประชุมระดมสมองผู้เชี่ยวชาญด้วยเทคนิค Nominal Group Technique และการศึกษานำร่อง วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา การวิเคราะห์เนื้อหา และ Paired t-test
ผลการวิจัยระยะที่ 1 พบว่าผู้สูงอายุร้อยละ 31.7 สงสัยมีภาวะสมองเสื่อม แกนนำครอบครัวมีความรู้ระดับต่ำ ทัศนคติเชิงลบ และทักษะต้องอบรม ระยะที่ 2 พัฒนานวัตกรรม 4C Model ประกอบด้วย (C1) Cognitive and Attitudinal Foundation (C2) Comprehensive Surveillance (C3) Competency-based Caregiving และ (C4) Connected Community through Technology จัดกิจกรรม 8 สัปดาห์ โดยบูรณาการการเรียนรู้แบบเพื่อนช่วยเพื่อน การใช้เทคโนโลยีท้องถิ่น และการดูแลผู้ดูแล ระยะที่ 3 พบว่าแกนนำครอบครัวมีความรู้เพิ่มขึ้น 7.74 คะแนน (95% CI: 6.74, 8.73; d=2.66) ทัศนคติเพิ่มขึ้น 1.49 คะแนน (95% CI: 1.23, 1.75; d=1.93) และทักษะเพิ่มขึ้น 1.61 คะแนน (95% CI: 1.34, 1.88; d=2.05) ทักษะคงอยู่ในระดับดีหลัง 3 เดือน และผู้เกี่ยวข้องมีความพึงพอใจร้อยละ 100
การวิจัยนี้ได้ผลลัพธ์เป็นที่น่าพึงพอใจในการพัฒนานวัตกรรม 4C Model ที่มีขนาดอิทธิพลสูงมาก (d=1.93-2.66) เหนือกว่าการศึกษาอื่น 2-5 เท่า มีความยั่งยืนและได้รับการยอมรับสูง นวัตกรรมนี้สามารถเป็นต้นแบบการพัฒนานโยบายการดูแลผู้สูงอายุระดับครอบครัวและชุมชน ช่วยลดภาระระบบสาธารณสุขและสร้างเครือข่ายดูแลสุขภาพอย่างยั่งยืน
