Abstract

The objectives of this study were to examine charge of care per day and structure of charge of
inpatients care. A research was performed at Buddhachinaraj Hospital by proportional sampling
3,133 cases from inpatients database of 2008. Data was collected from inpatient medical records
together with diagnosis related group (DRG) from electronic database. Samples were divided into 2
groups according to medical DRG and surgical DRG and each group was classified for length of
stay (low outlier, inlier and high outlier).
The results showed that the highest daily charge of care in low outlier of surgical DRGs was on
the first day of admission. The average was 11,877 baht and accounted for 37.8 percent of the
average charge of inlier group. The highest daily charge of care in high outlier of medical DRGs was
on the first day of admission. The average was 4,088 baht and accounted for 36.1 percent of average
charge of inlier group. The highest proportions of care in medical DRG were for drugs and intravenous
substances that accounted for 25.0 percent. The proportions in low outlier, inlier, and high
outlier were 20.6 percent, 23.7 percent, and 26.9 percent, respectively. The highest proportions of
care in surgical DRG were for procedures and anesthetic that accounted for 25.0 percent. The proportions
in low outlier, inlier, and high outlier were 30.1 percent, 27.0 percent, and 19.7 percent,
respectively.
The results of this study provide data on daily charge of care and the structure of charge. These
findings are useful in estimating cost and managing efficiency and quality of hospital care.
