DEVELOPING A MODEL OF SELF-EFFICACY COMBINED WITH SOCIAL SUPPORT THROUGH LONG-DISTANCE HOME VISITS FOR CHRONIC NON-COMMUNICABLE DISEASE PATIENTS AMONG VILLAGE HEALTH VOLUNTEERS THROUGH COMMUNITY PARTICIPATION

This research and development (R&D) study aimed to: 1) examine the context and problems related to home visits for non-communicable disease (NCD) patients, 2) develop and pilot a self-efficacy and social support model for long-distance home visits among Village Health Volunteers (VHVs) through community participation, and 3) evaluate the model's outcomes. The study was conducted in three phases. Phase 1 involved context and problem analysis, with a quantitative sample of 264 VHVs and 12 health network partners as qualitative informants. Phase 2 focused on model development and implementation using the Appreciation-Influence-Control (A-I-C) participatory technique and the LINE application. Phase 3 involved testing and evaluating the model with 30 VHVs over a 12-week period. Research instruments included a general information questionnaire, a self-efficacy scale, and a social support scale. Data were analyzed using descriptive statistics, the Wilcoxon Signed Rank Test, and content analysis.
Phase 1 results revealed that VHVs had moderate overall self-efficacy (Mean = 3.06) and social support (Mean = 3.12). Key problems identified were the absence of systematic long-distance home visit guidelines and insufficient communication equipment. The developed VHV-SSS Care Model comprises seven operational steps: 1) scheduling and equipment preparation, 2) patient assessment, 3) health data evaluation, 4) lifestyle counseling based on the 3E2R principle, 5) basic medication guidance, 6) complication surveillance guidance, and 7) documentation and data referral to the hospital. After piloting the model, VHVs' mean scores for self-efficacy (4.35) and social support (4.28) were significantly higher than before the intervention (p < 0.001). In conclusion, the model developed through community participation effectively enhanced VHVs' performance in providing systematic care for NCD patients, reduced distance and time barriers, and ensured continuity of patient care. Public health agencies should adopt this model as a standard guideline for long-distance home visits to improve primary healthcare services in other areas with similar contexts.
