Comparison of Recurrence Rates of Pterygium Between Bare Sclera Technique (BST) and Conjunctival Autograft Transplantation (CAT) at Aranyaprathet Hospital

Abstract

Background: Pterygium is a conjunctival disorder commonly found in tropical countries, and postoperative recurrence remains a significant clinical problem. Commonly used surgical techniques include the Bare Sclera Technique (BST) and Conjunctival Autograft Transplantation (CAT), which have been reported to have different recurrence rates.

Objectives: To compare the recurrence rates of pterygium within 6 months after surgery between the Bare Sclera Technique and Conjunctival Autograft Transplantation, and to analyze factors associated with pterygium recurrence.

Methods: This retrospective analytical study included patients with pterygium who underwent pterygium surgery at Aranyaprathet Hospital between January 1, 2022, and December 31, 2024. A total of 100 patients were enrolled, with 50 patients in the BST group and 50 patients in the CAT group. Data were analyzed using descriptive statistics, the Chi-square test or Fisher’s exact test, and univariate and multivariate logistic regression analyses.

Results: Patients in the BST group had a significantly higher mean age than those in the CAT group (p-value < 0.01), while other baseline characteristics did not differ significantly between the groups. The recurrence rate of pterygium was significantly higher in the BST group than in the CAT group (28.0% vs. 4.0%, p-value < 0.01). Postoperative complication rates did not differ between the two groups. Multivariate logistic regression analysis showed that the Bare Sclera Technique was an independent risk factor for pterygium recurrence (Adjusted OR = 8.43; 95% CI 1.66–42.82; p-value = 0.010)

Conclusion: Pterygium surgery using Conjunctival Autograft Transplantation resulted in a significantly lower recurrence rate compared with the Bare Sclera Technique.
