รายงานกรณีศึกษาการผ่าตัดถุงน้ำชนิดโอดอนโตเจนนิกเคอราโตซิสต์ (Odontogenic Keratocyst) หลายตำแหน่งที่กระดูกขากรรไกร

ผู้ป่วยหญิงไทย อายุ 21 ปี ถูกส่งตัวมาพบทันตแพทย์ที่กลุ่มงานทันตกรรม โรงพยาบาลอุทัยธานี ด้วยพบถุงน้ำหลายตำแหน่งในกระดูกขากรรไกรจากภาพถ่ายรังสีพาโนรามิก ด้วยผู้ป่วยต้องการจัดฟัน เนื่องจากฟันน้ำนมหลุดแล้ว แต่ฟันแท้ไม่ขึ้น ได้รับการตรวจภายนอกช่องปาก บริเวณฝ่ามือพบรอยจุดหลายตำแหน่งบนผิวหนัง รอยจุดมีขนาดเล็ก มีเส้นผ่านศูนย์กลางประมาณ 1-2 มม. ตรวจภายในช่องปาก ตรวจไม่พบฟันซี่ 18, 26, 28, 37, 38, 46, 48 จากภาพรังสีพาโนรามิกพบมีเงาโปร่งรังสี (Radiolucent) ขนาดใหญ่ ขอบเขตชัดเจน (Well-defined margin) หลายตำแหน่งทั้งขากรรไกรบนและขากรรไกรล่างส่วนหลัง ภาพรังสีกะโหลกศีรษะ (PA skull) พบเงาทึบรังสี (Radiopaque) ที่เยื่อหุ้มสมองบริเวณตรงกลางของกะโหลกศีรษะ ผู้ป่วยได้รับการรักษาโดยการผ่าระบายโพรงให้เป็นทางเปิดสู่ภายนอก (Marsupialization) ร่วมกับการผ่าตัดชิ้นเนื้อเพื่อวินิจฉัย (Biopsy) ที่ขากรรไกรล่างด้านขวา และซ้ายภายใต้ยาชาเฉพาะที่ จากลักษณะทางจุลพยาธิวิทยาได้รับการวินิจฉัยเป็นถุงน้ำชนิดโอดอนโตเจนนิกเคอราโตซิสต์ (Odontogenic Keratocyst) จากการตรวจทางคลินิก การตรวจทางภาพรังสี และการตรวจทางจุลพยาธิวิทยาของผู้ป่วยเข้าได้กับนีวอยด์เบซัลเซลล์คาร์ซิโนมาซินโดรม ผู้ป่วยได้รับการรักษาโดยการควักถุงน้ำออกทั้งหมด (Enucleation) ทั้งขากรรไกรบนและล่าง หลังการผ่าระบายโพรงให้เป็นทางเปิดสู่ภายนอกเป็นเวลา 10 เดือน ภายหลังการรักษาผู้ป่วยหายเป็นปกติ ไม่พบภาวะแทรกซ้อน และติดตามการรักษาเป็นเวลา 2 ปี ไม่พบการกลับมาเป็นซ้ำ
