ภาวะสารน้ำในช่องเยื่อหุ้มของผู้ป่วยโรคตับแข็ง

ภาวะ hepatic hydrothorax คือภาวะที่มีสารน้ำในช่องเยื่อหุ้มปอดมากกว่า 500 มล. ในผู้ป่วยโรคตับแข็งที่มีภาวะ portal hypertension โดยไม่ได้มีสาเหตุมาจากโรคปอด โรคเยื่อหุ้มปอด หรือโรคหัวใจ ภาวะนี้มีอุบัติการณ์ร้อยละ 5-15 และมักพบได้บ่อยในผู้ป่วยตับแข็งที่มีน้ำในช่องท้อง มีภาวะไตวายเฉียบพลัน มีภาวะ hepatic encephalopathy ร่วมด้วยซึ่งสัมพันธ์กับความรุนแรงของโรคและอัตราการเสียชีวิตที่เพิ่มขึ้น ผู้ป่วยที่มีภาวะ hepatic hydrothorax จะมีอัตราการอยู่รอดเฉลี่ยอยู่ที่ 8-12 เดือน อาการของโรคมีตั้งแต่อาการเล็กน้อยไปจนถึงรุนแรงมากจนทำให้มีระบบทางเดินหายใจล้มเหลวได้ขึ้นกับปริมาณน้ำในเยื่อหู้มปอด การรักษาภาวะนี้ทำได้ยาก เนื่องจากผู้ป่วยมักมีโรคร่วมและเสี่ยงต่อการเกิดภาวะแทรกซ้อนจากการรักษา ดังนั้นการรักษามุ่งเน้นเป็นการรักษาแบบสหสาขา ในบทความนี้มุ่งทบทวนอาการทางคลินิก การวินิจฉัย และการรักษาในปัจจุบัน
