Development of Nursing Practices Guideline for Cholangiocarcinoma patients in Critical Condition after Liver Resection, Intensive Care Ward, Sakon Nakhon Hospital

Abstract

         This development study aimed to 1) develop the clinical nursing practice guideline (CNPG) for cholangiocarcinoma patients in critical condition after liver resection 2) study the level of feasibility of this CNPG implementation and 3) study the satisfaction of nurse for implementation of the CNPG. The Soukup’s evidence–based approach was applied during the process of CNPG development consisting of 4 stages: 1) clinical problem investigation 2) evidence–based practice investigation 3) CNPG development and trying out and 4) CNPG implementation. This study was completed in the third stage in which twelve relevant studies were selected. The knowledge was extracted and then the 9 aspects of CNPG for cholangiocarcinoma patients in critical condition after liver resection was developed according to the conceptual framework of FANCAS, FASTHUG and BANDAIDS for critical patient evaluation. The content validity index and reliability tested by the Cronbach’s alpha coefficient of this drafted CNPG were 0.85 and 0.80, respectively. The drafted CNPG was applied by 14 registered nurses who had experiences in surgery ICU at least 1 year, from September to October 2019. The results showed that the feasibility and satisfactory of CNPG application were in the highest level (4.78, S.D. = 0.02) and (4.61, S.D. = 0.02), respectively.

         In conclusion, this clinical nursing practice guideline for cholangiocarcinoma patients in critical condition after liver resection in surgery ICU, Sakon Nakhon hospital could be applied in practical and develop continuously.
