Development of Clinical Nursing Practice Guideline for Asthmatic Children at Sakon Nakhon Hospital

Abstract

            This developmental research aimed to develop clinical nursing practice guideline (CNPG) for pediatric patients with asthma in Sakon Nakhon hospital from April to September, 2020. The process of CNPG development was conducted by applying the conceptual framework of the Australian National Council for Medical and Public Health Research. Four evidence–based practices retrieved from the medical database and related research articles were studied. The developed CNPG was verified for accuracy by 4 experts. The quality of CNPG was evaluated by the AGREE II. The study samples were divided into 2 groups, 23 pediatric patients with asthma aged 1–15 years who admitted to the pediatric ward, Sakon Nakhon hospital and 16 registered nurses who use the CNPG for pediatric patients with asthma. The data collection tools were the questionnaire for sample demographic, feasibility assessment form for CNPG usage, observation form for CNPG compliance and respiratory failure assessment form. Data were analyzed using descriptive statistics.

            The results showed that the developed CNPG consisted of 4 components; 1) assessment of the asthma severity 2) nursing care for patients on oxygen supplement 3) Drug administration by nebulization and 4) Discharge planning for asthmatic patients. It was found that the feasibility of CNPG application was 93.1%. The CNPG compliance was 92.6%. The incidence of respiratory failure according to recurrent asthma attacks was decrease from 19.6% to 4.3%.

            This study suggests that the CNPG should be applied to pediatric patients with other respiratory diseases for further development of nursing service quality.
